Questions to ask

  • Was the original assessment supported by enough evidence?
  • Were the test location and observation time appropriate?
  • Were too many variables changed together?
  • Did treatment stop promptly when a problem appeared?

Review sequence

  1. Reconstruct the timeline.
  2. Separate immediate causes from underlying process problems.
  3. Define improvements that can be checked.
  4. Update the checklist and arrange targeted retraining.

Do not conceal failures or look for a scapegoat. Where a customer's property is involved, prioritise transparent communication and responsible resolution.
